What is perioral dermatitis?
Although perioral dermatitis is usually treated by dermatologists we wanted to shed some light on the issue and provide alternatives for those suffering with it.
Perioral dermatitis is a chronic cutaneous (outer skin) affliction characterized by the appearance of erythematous (red), inflamed papules and pustules around the mouth and less frequently around nostrils, eyes and forehead. Itching, stinging and burning sensations can also be present along with eczematous patches.
Some experts classify it as a type of acneiform eruption; it resembles acne but it is not acne per se.
The condition occurs most commonly in young women, but can also affect men and children.
An exact cause is still unknown but possible contributory factors include:
- Prolonged application of topical steroids to treat other minor skin conditions;
- Use of fluoridated toothpastes;
- Continuous irritation with pH-disturbing/harsh cleansers, and shampoos; rich moisturizers, lotions; fragrances (including essential oils derived ingredients) and colorants.
- Internally, it could be caused by hormonal imbalances, imbalances in the intestinal flora, low hydrochloric acid levels and/or consumption of gluten-containing foods and dairy.
Conventional treatment is prescription of oral and, sometimes, topical antibiotics, such as benzyl peroxide, erythromycin, clindamycin, metronidazole, etc – often for several months to prevent recurrent breakouts.
Naturopathic doctors opt to prescribe natural antibiotics and replenish the intestinal flora with high-strength probiotics. They would also advise discontinuing all steroid medications (this could trigger a healing crisis), harsh/heavy cosmetics as well as fluorinated toothpastes. Oily or thick moisturizers and comedogenic should also be avoided.
A diet rich in fresh fruits, vegetables and omega fatty acids (to reduce inflammation) and devoid of processed foods, dairy and gluten has proven to support the healing process.

The patient’s experience: “I have had perioral dermatitis for several years.
